Certified copies of your degree certificate(s) and academic record (if you attended or are currently a student at SMU or another university).
Certified copy of your marriage certificate (if applicable).

Certified copies of the following documents:
Matric exemption certificate (including subject marks)
ID document / Passport / Work permit / Study permit

Comprehensive academic record (indicating marks obtained for subjects)
Certificate of registration with the relevant professions council (if applicable)
Letter of appointment as a registrar with a date of assumption of duty: applicants for M Med and M Dent students NB: letter from the employer, e.g. Hospital HR.
Motivational letter and CV (Public Health applicants only)
Medical aid (Non-South Africans)
Students with non-South African qualifications must attach a SAQA evaluation certificate with their previous degrees obtained

To apply online, follow the steps below:

Select “Click here to apply.”
Accept the application rules and click “proceed.”
Enter your details and click “next.”
Enter academic scores, add subjects, then click “next.”
Select your school from the drop-down box and click “next.”
Choose two program options, click “add qualification,” and then “next.”
Verify application details and click “Continue.”
To upload documents, select “yes” and click “next.”
Click “Load/View Document,” then “Upload Document,” choose files, and save.
To finish uploading all documents, click “Finish upload.”
After completing the application process, you will receive a confirmation page
